The best advice to give your Doctor's team is to get a development consultant to lead the project. I don't believe it is possible for "laymen" to develop anything anymore. For an architect to take on that responsibility or to presume their client will make his deadlines, and attend to site development regulations, is unrealistic. A D/B typically is not better qualified either.
Development is no country for laymen, as they say.
You would do yourself and the client well to get them good advice and get an expert on the team.
