This may be a longshot, (long distance), but you might try EggRock modular bathrooms in Littleton, MA, http://www.eggrockmodular.com/index.html  

I haven't used their products, but I've toured their factory and they do extremely high-quality bathrooms, and will customize their system to your design.  Most of their work is for hotel chains, but they can do just about anything.

Dwyer.  They are commercial, and often used in hospital work.  You might ask them if they have something more residential oriented.
